Concordia Plan Services is introducing a new program to
complement our Be Well … Serve Well health and wellness
initiative.
The new program is designed to encourage Concordia
Health Plan (CHP) members to take the next steps toward
living healthier lives.
The new Be Well Rewards
program will run from May 1 – December 31, 2011. It
offers CHP workers and enrolled spouses up to $250 for
engaging in healthy behaviors. The Be Well Rewards
program features a menu of healthy activities these
members can complete. Each activity is worth 25-100
points, and members can earn up to 250 points. Points
convert to dollars on a debit card or store gift card,
up to $250 per person.
